Background

Typographical errors may either be the result of recording incorrect data as placed on the requisition form or a secretary transcribing an incorrect word or phrase. The result is often amusing and unexpected. The following phrases are taken from actual reports (before being finalized)!

Diagnosis

Erythema nervosum (for erythema nodosum)

Submitted by a veterinary pathologist...I dictated numerous erythematous skin lesions in the ventral abdomen of a pig and I said photos, meaning I took pictures. The secretary transcribed it as four toes.
